Course Registration Form
To be completed by each student prior to starting any program or course.  The completed form must be accompanied by the Registration Fee*.

Student Contract

Student Declaration Form
Complete this form only if taking a program that requires Body Work. Click the above title link to contact the BC Institute of Holistic Studies regarding this form. A completed Student Declaration should be sent in together with a completed Student Contract and Course Registration Form.

English Language Declaration 
A completed English Language Declaration form must accompany the Course Registration and Student Declaration forms, as well as the Receipt for Courses and Policies booklet form.
